ITEM 18.a: The following statements, although set forth here in the text box for Question 18.a., pertain not to Question 18.a. but rather to Questions 10 and 17, respectively. The statements are set forth here because the online version of Form LM-1 does not allow for any explanatory text to be set forth in the response boxes for Questions 10 and 17; and this box for Question 18.a. presents the best opportunity to insert explanatory text in regard to Questions 10 and 17. Question 10: SIA's approximately 4150 Members are based at 19 cities across the United States (including Salt Lake City, UT) and they work all over the country. As indicated in SIA's Constitution & Bylaws, its operations are co-extensive, and its physical address is in Salt Lake City, Utah. Question 17 is objectionable in that it is built a false premise and presents a logical fallacy. Specifically, it requires that SIA--which has no "parent" organization-- answer either or "yes" or "no" as to whether its (non-existent) "parent" filed a constitution and bylaws on its behalf. Apparently the online filing system will not permit this Form LM-1 to be submitted unless either "yes" or "no" is selected in response to Question 17; accordingly, SIA has selected "no." The fact remains that SIA has no parent organization, and it has no affiliates or subsidiaries. Rather, as is reflected in its Constitution & Bylaws, which is being filed with this Form LM-1, SIA is simply a national labor organization that is independent of any other organization.
 
ITEM 18.b: SkyWest InFlight Association ("SIA"), an unaffiliated labor organization which has not parent and no subsidiaries, in the interest of its Members does not levy assessments and has no plans to do so.
 
ITEM 18.c: SIA has successfully negotiated with the Employer for insurance and benefit plans for SIA's Members; SIA intends to maintain and continue to improve upon such negotiated-for terms and conditions and does not require or participate in any plans for itself.
 
ITEM 18.d: SIA generally has no funds. Section 17 addresses the hypothetical possibility that SIA might have any funds in the future.
 
ITEM 18.e: SIA collects no funds, has no funds, does not conduct financial transactions, and is not subject to audit.
 
ITEM 18.g1: SIA does not have stewards, is not affiliated with any other bodies composed of labor organizations or labor representatives,, and does not select representatives to or for any such bodies.
