Prefill Date Explanation:See item 3 for an explanation.Item 3 Terminal: The union has been terminated and the assets were transferred to OPEIU Local #153. The union pin number 019065. OPEIU Local #112 isn't feasible to continue to operate due to the Janus ruling and loss of members and income. The address of the LM-2 has been changed to reflect OPEIU Local #153. All records were transferred to the address listed on Form LM-2. OPEIU Local #153 address is 42 Broadway, Suite 1201 New York, NY 10004.

Cash was transferred in the amount of $881,669 to OPEIU Local #153.

Question 12: Kline and O'Hay LLC

Question 15: The organization dissolved and wrote a check to OPEIU Local 153 in the amount of $881,669. The organization continues to lose money and with the Janus decision the organization opted to dissolve and transfer assets to OPEIU Local 153.
