IT WAS CREATED TO PROVIDE VISION, DENTAL AND SUPPLEMENTAL LIFE INSURANCE TO THE STATE WORKERS. A COPY OF THE TRUST'S AUDITED FINANCIAL STATEMENT IS ATTACHED. DISTRICT 1199 DID NOT CONTRIBUTE TO THIS TRUST AND THE AMOUNT OF THE CONTRIBUTION MADE BY THE STATE ON THE DISTRICT'S BEHALF IS NOT DETERMINED DUE TO THE OTHER UNIONS INVOLVEMENT.
